Shortest Forms
The following texts have been extracted from the ritual books for use in only the most extreme necessity, such as at the point of death. At other times, according to the circumstances, the minister should add other prayers, or the complete rite, according to the provisions and norms of the liturgical books

In a case of necessity any person with the right intention, confers baptism licitly (canon 861.2)
For Children:
(Rite for Baptism of Children in danger of death, n. 164) At the moment of death, it is sufficient for the minister to omit all other ceremonies and pour water on the child while saying the usual words:
N., I baptize you in the name of the Father, and of the Son and of the Holy Spirit.
N., Ego te baptizo in nomine Patris, et Filii, et Spiritus Sancti.
It is desirable that the minister, as far as possible, should use one or two witnesses.
For adults:
(Christian Initiation in Danger of death, n. 373) In the case of a person who is at the point of death, that is, whose death is imminent and time is short, the minister, omitting everything else, pours natural water (even if not blessed) on the head of the sick person while reciting the usual sacramental form
N., I baptize you in the name of the Father, and of the Son and of the Holy Spirit.
N., Ego te baptizo in nomine Patris, et Filii, et Spiritus Sancti.
Conditional baptism may be administered using the formula “If you are alive, I baptize…” in cases where it is doubtful if the person is alive; or “If you were not baptized, I baptize….” in cases where one has a legitimate doubt as to whether the person has been baptized.
(Christian Initiation in Danger of Death, n. 291) In case of necessity, it is enough to anoint with chrism, while saying the words:
N., be sealed with the Gift of the Holy Spirit.
N., Accipe signaculum doni Spiritus Sancti
(Rite of Penance, n. 21) In imminent danger of death is it sufficient for the priest to say the essential words of the form of absolution, namely
I absolve you from your sins, in the name of the Father, and of the Son, + and of the Holy Spirit.
Ego te absolvo a peccatis tuis in nomine Patris, et Filii, + et Spiritus Sancti.
APOSTOLIC BLESSING FOR THE DYING

Through the holy mysteries of our redemption, may almighty God release you from all punishments in this life and in the life to come. May he open to you the gates of paradise and welcome you to everlasting joy. R. Amen.
Per sancrosáncta humánæ reparatiónis mystéria, remíttat tibi omnípotens Deus omnes præséntis et futúræ vitæ pœnas, paradísi portas apériat et ad gáudia te sempitérna perdúcat. R. Amen.
[If the oil is not blessed, the priest blesses it, saying:
Bless, + Lord, your gift of oil and our brother/sister N. that it may bring him/her relief.
Bénedic, + Dómine, hanc Ólei creatúram et ipsum étiam infírmum (ipsam étiam infírmam) qui (quæ) hoc leniménto perúngitur. ]
The priest anoints the sick person with the blessed oil
First he anoints the forehead saying:
Through this holy anointing may the Lord in his love and mercy help you with the grace of the Holy Spirit. R. Amen.
Per istam sanctam Unctiónem et suam piíssimam misericórdiam, ádiuvet te Dóminus grátia Spíritus Sancti. R. Amen.
Then he anoints the hands, saying:
May the Lord who frees you from sin save you and raise you up. R. Amen.
Ut a peccátis liberátum (liberátam) te salvet atque propítius állevet. R. Amen.
The sacramental form is said only once, for the anointing of the forehead and hands, and is not repeated. In case of necessity it is sufficient that a single anointing be given on the forehead, or, because of the particular condition of the sick person, on another suitable part of the body, while the whole sacramental formula is said
The minister goes to the sick person, and showing the Blessed Sacrament, says:
The Body of Christ. R. Amen.
Corpus Christi. R. Amen.
[Then the minister says:
The Blood of Christ. R. Amen.
Sanguis Christi. R. Amen. ]
Immediately, or after giving communion to the sick person, the minister adds:
May the Lord Jesus Christ protect you and lead you to eternal life.
R. Amen.
Ipse te custódiat et perdúcat in vitam ætérnam.
